Death at a Private Address Copy

If the deceased person has died at a private address, in addition to the essential information on the first call checklist, it is recommended that a funeral director also ask these additional questions:

  1. Has a doctor or a person qualified to confirm death (e.g. a paramedic) attended?
  2. The name of the doctor or person qualified to confirm death (if appropriate)
  3. Confirmation that the doctor or person qualified to confirm death has given permission for the deceased person
    to be moved.
  4. Are the family ready for the deceased to be moved to the funeral home.
