Unit 4 – Essential information

It is recommended that as a minimum the funeral director captures the following information during the first call, a checklist can be used – often called a ‘first call sheet’.

A first call sheet will typically contain the following points:

  • the caller’s name;
  • the caller’s telephone number;
  • the deceased’s full name;
  • the location of the deceased;
  • Has the doctor or a person qualified to confirm death attended (if appropriate)?

Funeral Directors should also use the first call sheet to note the date, time and name of the person who took the first call.
